All City officers and employees who are required to file Statements of Economic Interests with the San Francisco Ethics Commission must file a copy of this form annually with the Ethics Commission at 25 Van Ness Avenue, Suite 220, San Francisco, CA 94102, no later than April 1st. A filer who assumes office after April 1st must file this form within 30 days of the date that he or she is sworn in or assumes employment. Ethics Commission Reg. 67.33-1. The completed forms are public records. We recommend that you retain a copy of your completed form for your records for at least five years. Questions regarding this form may be directed to the Ethics Commission.
